Minutes of meeting held 25.2.08. at Chelmsford Golf Club, Chelmsford, Essex.

Matters arising : Nil.

Treasurer - £76K carried forward to 2008 (£100K in last year + arrears bulk payments). Jan-Jan accounting now. Monthly income ~ £5K.

Chairman - Nil.

Secretarial - OHAG : Running well, apart from SE (although showing some interest), with SW and Mid discussing increasing low UDA values to PCT average.

                    - Audit : Infection control adopted by NE/SW/W (dentists collect data and PCT interprets). Record keeping in Brentwood.

Capital Funding and Commissioning Changes : Mark Shackell outlined 'Stairway to Heaven?', where the key drivers are world class commissioning (Gateway 8754 Dec 2007), NHS operating framework (Gateway 9120 Dec 2007), E of England commissioning strategy, DH commissioning NHS dentistry guidance - series, and PCC advice on balanced scorecards (QOF - quality and outcomes framework). This 'smart commissioning' is intended to bring private sector methods and process to the NHS in order to drive up choice, productivity, quality and value for money. PCTs are appointing 'commercial services' managers who will use NHS logistics to draw up the tender documentation. For bigger contract values the OJEU (nee OJEC) competition rules probably apply (>£600K). The CDO is now a policy adviser, although he will still be trumped by finance and commissioning (11% increase). The emphasis is shifting from access to access with quality end points, with systematic review against locally agreed standards (including analysis of patients/practices/complaints and 'mystery shopper' exercises to validate information from practices). Commissioning plan by end of 2008, which will include increased UDAs to selected practices/removing caps and opening new practices.   

End of year monitoring : Some PCTs flexible in first year allowing up to 10% over/underproduction; others sticking to 4% tolerance and refusing to contemplate 'local commissioning' for individual cases (noteably SE again). Lower UDA values being looked at, but at what cost to standard of dentistry delivered? Child only contracts to cease, unless PCT cannot find alternative.     

Essex Univ. Foundation course : Problems, with complaints from students (poorly selected by univ.), not enough 'hands on', and no strong individual running things. Course will continue, but needs PCT funding and more input from dentists re programme/interviewing candidates, etc.

VT/Extended training practices : ?funding from PCTs for 2/3 years and 2 VDPs.

Ortho : 18 weeks focus. Spencer Nute covers whole of S and sees new patients within 2 weeks, with only 7 on waiting list (colleague has ~30). Concerns about patients travelling from outside, and failure of other PCTs to contribute.

LDC Conference : Dinner Thursday 12th June/Conference 13th June (5 members). Motions invited.

LDC Constitution : NE/Mid happy, but silence elsewhere.

Website : Some confusion with passwords (digital certificate -encrypted passwords-difficult to organise). Simple instructions to be demonstrated at next meeting by cyber experts Piers Lambert and Brian Harvey.
